HomeHealth
HomeHealth services are designed for ill or injured patients who no longer require hospitalization, but have difficulty or are unable to leave home to receive the care they need. In essence, we bring hospital care to the comforts of your own home! For many, it’s an excellent alternative to extended stays in the hospital or placement into a nursing home. Schoolcraft Memorial HomeCare is State certified and is proud to provide to our community and surrounding areas the following:

Hospice
Hospice provides compassionate care and support services for patients and their family. The central goal of hospice is to allow patients with life limiting illnesses to continue their life free of pain and to manage other symptoms so that they can live their life with dignity, respect and purpose.
Hospice care focuses on the person not the disease. We take a team approach with our patients and their family. Hospice program may provide:
• Registered Nurses
• Aides
• Counseling
• Volunteer Services
• Social work Services
• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Medication
• Medical Equipment
• Respite Care (providing a break for the caregiver)
• Inpatient Hospital Care
• Bereavement Support Services
• Spiritual Care
Private Duty
We provide Private Duty Aide services to help our patients and their families with basic daily living needs.
Services include:
• housework
• shopping
• meal preparation
• bathing
• nail clipping
• transportation
• Private pay nursing and or therapy services are also available
• respite & companionship
We also offer Lifeline - a personal emergency response system that allows a person to summon emergency care 24-hours a day, seven days a week.
How do we qualify?
Home Health
• Physician’s Order
• Require a skilled need i.e. nursing, physical therapy etc.
• Homebound - leaving home requires much forethought and planning and or assistance.
Hospice
To qualify for hospice care, a patient’s physician must certify terminal illness with a life expectancy of less than six months. You must have have a 24-hour care giver.
Private Duty
Anyone can hire Private Duty Services.

PAYMENT INFORMATION
HomeCare: Medicare, Medicaid, BC/BS, other insurances, and private pay.
Hospice: Medicare, medicaid, BC/BS, other insurances.
Private Duty Services: Hourly rates for services.Services are billed directly to the client or family on a monthly basis.
